How to Register


Anyone in England can register with a GP surgery. 

You do not need proof of address or immigration status, ID or an NHS number.

Due to the ongoing Coronavirus Pandemic we are strongly encouraging patients to register online where possible, and speak with one of the reception staff if you have any queries.  The registration form can be found under the pre-registration tab, or  You can download a GMS1 registration form on GOV.UK

If you need help to fill in forms or you are not able to register online, call the surgery and let us know.

We will need your previous and current postcodes, and details of your previous GP, if you have one, before we can register you.

We are discouraging all footfall to the Practice with the exception of pre-booked, medically necessary face to face appointments.

You will need to complete a form and arrange an appointment for a new patient health check to start the registration process. You will be given a Practice Leaflet and a medical questionnaire which you need to complete and bring with you for your new patient health check.

It can take several weeks for transfer of your medical records so it is important you supply the basic information requested on the medical questionnaire.

Named, Accountable GP for all Patients

Building on commitments to deliver more personalised care it is a requirement for all patients to be allocated a named accountable GP with effect from 1 April 2015.

The named accountable GP will take lead responsibility for the co-ordination of all appropriate services and ensure they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).

The partners of The South Lawn Medical Practice have personal lists and every patient registered with the practice will already have a named accountable GP.

Newly registered patients will be allocated a named GP when they register.

Existing patients who move outside the Practice area will need to register with their new local Practice.
