How to Register

registrationPlease call in at reception bringing your medical card with you if you have one.  If you do not have a medical card please bring some other documents to confirm your identity, eg passport and utility bill. We will need your previous and current postcodes before we can register you.  

You will need to complete a form and arrange an appointment for a new patient health check to start the registration process. You will be given a Practice Leaflet and a medical questionnaire which you need to complete and bring with you for your new patient health check.

It can take several weeks for transfer of your medical records so it is important you supply the basic information requested on the medical questionnaire.

 

Named, Accountable GP for all Patients

Building on commitments to deliver more personalised care it is a requirement for all patients to be allocated a named accountable GP with effect from 1 April 2015.

The named accountable GP will take lead responsibility for the co-ordination of all appropriate services and ensure they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).


The partners of The South Lawn Medical Practice have personal lists and every patient registered with the practice will already have a named accountable GP.

Newly registered patients will be allocated a named GP when they register.

 

Practice Area

The Practice accepts new patients -from within the City Boundary East of the River Exe, excluding Topsham, a map of the practice area may be viewed in in the Practice showing the precise area area from which new patients may be accepted on to the list.  Or, follow this useful link to view the catchment area for the Practice, and check whether your postcode falls within this area: this is awaiting updating April 2019

Existing patients who move outside the Practice area will need to register with their new local Practice.
